General terms of Revive Whole Body Studio
I confirm and agree that the following terms and conditions and undertakings apply to the Exercises and Training to be provided to me by Revive Whole Body Studio (Pty) Ltd.
I have been examined by a licenced medical practitioner within the past 3 months and have been found by such practitioner to be able to perform all vigorous stretching and the Exercises which I am able to perform during my enrolment with Revive Whole Body Studio (Pty) Ltd.
I will faithfully follow all instructions given to me by Revive Whole Body Studio (Pty) Ltd and its teachers, as to when, where and how to perform and not perform the Exercises.
I agree and acknowledge that participation in any Exercises could constitute a risk of serious injury to myself including permanent paralysis or death. I voluntary and knowingly recognise, accept and assume this risk and warrant that I am physically fit and able to perform the Exercises without risking serious injury, including permanent paralysis and death. I acknowledge the neither Revive Whole Body Studio (Pty) Ltd, its owners, teachers or employees shall be, nor deemed to be responsible or liable (whether in contract or in tort or any other statue whatsoever) for any injury, illness or other mishap I sustain from or out of, or in any way directly or indirectly connected with the Exercises.

I indemnify and will at all times thereafter well and sufficiently indemnify and keep fully indemnified Revive Whole Body Studio (Pty) Ltd, it’s owners, teachers or employees ofrom and against all actions, suits, causes of action, proceedings, claims, costs and expenses whatsoever which may be taken or made against Revive Whole Body Studio (Pty) Ltd, its owners, teachers or employees in connection with or arising out of any injury, illness or mishap to me.
The tuition paid by me under this enrolment form to Revive Whole Body Studio (Pty) Ltd is non-refundable. Revive Whole Body Studio (Pty) Ltd may in its sole discretion grant refunds to me without prejudicing any of its rights., its owners, teachers and employees of Revive Whole Body Studio (Pty) Ltd are not liable for, nor expected to, provide any advice, training, or medical assistance other than in the form of the Exercises or workshops taught.
The tuition paid by me under this enrolment form to Revive Whole Body Studio (Pty) Ltd is non-refundable. Revive Whole Body Studio (Pty) Ltd may in its sole discretion grant refunds to me without prejudicing any of its rights.
