General terms of Yoga with Betsy
1) Liability Agreement
I fully understand and agree to the following:
When I am participating in yoga or any classes, workshops and/or private sessions offered by Yoga with Betsy whether in-person, online live or pre-recorded., I recognize that any physical exertion may be strenuous and may cause injury. I am fully aware of the risks and hazards involved.
I understand it is my responsibility to consult with a physician prior to and regarding my participation in these classes.
I am capable of judging the limits of my physical abilities and I understand it is my responsibility to stay within my range of ability.
I recognize it is my responsibility to notify my teacher of any serious injury or illness before every yoga class (whether in-person, live online or pre-recorded), workshop or private session.
In consideration of being permitted to participate in the classes (whether in-person, live online or pre-recorded), workshops and/or private sessions, I agree to assume full responsibility for any risks, injuries, illness or damages, known or unknown, which I might incur as a result of participating in any program offered by Yoga with Betsy.
I knowingly and voluntarily waive any claim I may have against Yoga with Betsy, and the Yoga with Betsy teachers, Staff and/or owner for injury or damages I may sustain as a result of participating in any classes (whether in-person, live online or pre-recorded), workshops, private sessions, or programs.
I have read the release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
2) Card on file
I authorize Yoga with Betsy to keep my credit card on file for any future transactions such as late cancellation and no show fees until I request a cancellation of my membership.
I understand that the terms of this authorization will remain in effect until I request that they be terminated. I acknowledge that I am responsible for making this request in writing.
3) Recurring charge
I understand that {if applicable} my auto-renewal membership will remain in effect until I request that it be terminated.
4) Membership changes, cancellations or hold policy:
I understand that Yoga with Betsy requires a minimum 30 days notice from next renewal date for changes, cancellations or holds on auto-renew memberships.
Membership holds apply in medical circumstances, for up to two months. This hold can ensure I keep my current membership rate when I come back.
I understand that a $24 hold fee per month applies.
In the event of a cancellation, I understand I forfeit my current rate when I decide to come back.
I understand my membership expires one month after my final payment.
5) Cancellation Policy
I understand that in the event of a late cancellation (less than 8 hours) I will be charged $15.
6) Failed Payments/Past Due Memberships
I understand I will be notified if my account payment fails to authorize for any reason. If not addressed within 24 hours, a $5 late fee will apply. If a valid payment is not made within 3 days of the original payment date, a $5 late fee will apply.
I understand that if my membership is 5 days past due, my membership will be suspended and I will not be allowed to participate in classes. Reinstatement requires payment of dues owed and a reinstatement fee of $10. Memberships will be canceled after being suspended for 15 days.
7) Drop-in Class Policy
I understand that if I cancel within 8 hours of class or do not show, I forfeit my paid for drop-class credit.
8) Face masks are not mandatory but will be left for each student or instructor to decide.
